Any HTML Form Wizards around?

Any HTML Form Wizards around? If so I have a broken HTML form at the following link that I need help fixing.

It won't send the required email address when all the correct information is entered.

If no one is around that can help that's fine too, just thought I would check.

I hesitate to call myself a wizard, but I couldn't help taking a quick look.

Aren't you missing an end form tag? (</form>)

There are (at least) two problems, one syntactic and one cgi-related.

Below, swap the parenthesis for angle brackets.

1) You are missing (/form)(/div)(/p)(/td) between the final (/fieldset) and (/tr), and
2) The form action attribute is empty - presumably it should be a (relative) uri specifying the appropriate cgi 'script' on the server to handle the form submission.

Given (2), I suspect you need some further guidance ... ?

Not sure but the INPUTs inside the form do need NAME="" tag, so server side can parse such arguments.

Each input field must have a name attribute to be submitted.

If the name attribute is omitted, the data of that input field will not be sent at all.
